Financial Policy:
I am a provider for a variety of insurance carriers and Employee Assistance Programs. Please contact your insurance carrier to confirm eligibility for mental health services and to obtain a treatment authorization number if required. My office will submit all billing claims and updated treatment plans.
You may choose to not utilize your mental health benefits. By paying "out of pocket" you and I have more flexibility regarding your treatment. Some insurance plans require extensive private information to determine "medical necessity for on-going treatment" and other companies will not cover couples/marital therapy without a diagnosis being assigned to the "primary patient".
You are responsible for your copay at each session. Payment may be made by cash, personal check or debit/credit card.
A $65.00 missed appointment fee is charged for appointments not canceled within 24 hours. This charge is solely your responsibility, as insurance companies do not cover a missed appointment.
Returned checks are subject to an additional $25.00 fee.
